With the popularity of the electronic medical record (EMR), doctors’ ability to write has declined. Virtual scribes and cookbook formulas have sadly replaced long form traditional dictation and writing. When I retired from Cardiology in 2015 over half of other doctors notes I read provided me with almost no insight or useful information about what the doctor thought or observed. I read my IBD doctors notes about me (I have Crohn’s disease), and found only a couple of relevant sentences out of two pages of repetitive gobble gook, or as we use to say computer vomit
